How Does TB-500 Work?

Published

TB-500 names two different molecules. Analytical papers identify it as containing the N-acetylated 7-residue fragment of thymosin beta-4, Ac-LKKTETQ (residues 17-23). The name is also used for full-length thymosin beta-4, a 43-residue peptide. The mechanism behind both is thymosin beta-4's role as the main G-actin-sequestering peptide in many cells: it binds actin monomers 1:1 and holds them out of filaments. Cell and animal studies link it to cell migration and tissue measures in wound and heart models.

Two molecules, one name

A 2012 analytical study detected and identified the N-terminal acetylated 17-23 fragment of human thymosin beta-4, Ac-LKKTETQ, in a TB-500 formulation using high-performance liquid chromatography with high-resolution mass spectrometry. The authors also synthesized the fragment by solid-phase synthesis as a reference, and described TB-500 as a formulation suspected of use as a doping agent in sport. In vitro · PMID 22962027

Outside that analytical literature, the name is also applied to full-length thymosin beta-4. A 2026 review describes thymosin beta-4 as the predominant beta-thymosin in mammalian cells and notes that its N-terminal metabolite Ac-SDKP has a literature of its own. Because one name covers molecules of very different size, the identity of any given sample is a question for analytical testing rather than for the label.Review · PMID 41570941In vitro · PMID 22962027

The core mechanism: sequestering G-actin

Actin, the protein that forms the cell's internal filaments, exists as free monomers (G-actin) and as polymerized filaments. A 1991 study found that at least 50% of the actin in resting human platelets is unpolymerized and mostly bound to an acidic, heat-stable 5-kDa peptide called Fx. Sequencing showed Fx to be identical to thymosin beta-4, first isolated from calf thymus and once thought to be a thymic hormone. In vitro · PMID 1999398

The same study showed that thymosin beta-4 binds muscle G-actin stoichiometrically, forming a 1:1 complex with actin monomers, and that this complex inhibits salt-induced polymerization. The authors concluded from its wide distribution and high intracellular concentration that it plays a significant role in regulating actin polymerization in many cell types. In vitro · PMID 1999398

A 1992 study measured the peptide in neutrophils (polymorphonuclear leukocytes) of human origin. It found a cytoplasmic concentration of about 149 microM and identified thymosin beta-4 as the major G-actin-sequestering protein in these cells. After chemoattractant stimulation, the amount of G-actin bound to thymosin beta-4 fell, consistent with the peptide being released as actin polymerizes. Its binding kinetics were fast enough to keep pace with the rapid actin changes seen in stimulated cells. In vitro · PMID 1447300

Cell migration and wound models

A 1999 rat study tested full-length thymosin beta-4 in a full-thickness skin wound model. Compared with saline, re-epithelialization was 42% greater at day 4 and up to 61% greater at day 7, and wounds contracted at least 11% more by day 7. More collagen deposition and angiogenesis were observed in the peptide-exposed wounds. In a Boyden chamber assay, keratinocyte migration was 2-3-fold higher than with medium alone after 4-5 h.Animal study · PMID 10469335

In cultured corneal epithelial cells of human origin, a 2004 study measured more than 2-fold increases in laminin-5 gamma2 chain and TGF-beta1 expression with thymosin beta-4. Blocking TGF-beta1 with neutralizing antibodies did not stop the laminin-5 response, and in TGF-beta1 knockout mice the response was larger than in wild-type mice, so the authors concluded the two act through independent pathways. In vitro · PMID 14729067

A 2026 rat study compared the two names directly in a tendon model: 32 rats had the Achilles tendon cut and repaired, in groups of eight. At four weeks, maximum load to failure was higher than control in the TB-500 group with statistical significance (p < 0.05), and total Bonar (p = 0.016) and Movin (p = 0.017) histology scores were significantly lower than control. The study described its TB-500 as synthetic thymosin beta-4.Animal study · PMID 42542926

Heart and liver models

A 2004 study described thymosin beta-4 as promoting migration of myocardial and endothelial cells in the embryonic heart, a property retained in postnatal heart muscle cells, and as raising survival of those cells in culture. It formed a functional complex with PINCH and integrin-linked kinase (ILK), activating the survival kinase Akt. After coronary artery ligation in mice, the authors reported higher ILK and Akt activity, greater early myocyte survival and better cardiac function measures.Animal study · PMID 15565145

Endogenous thymosin beta-4 has also been studied as a driver of fibrosis. In a 2023 mouse study, deleting it selectively in activated hepatic stellate cells reduced liver injury and fibrosis in two fibrosis models by repressing Hedgehog signaling, and re-expressing it reversed that effect. This points in the opposite direction from the wound studies, which is why the 2026 kidney review calls for frameworks that consider effects on fibrosis in both directions.Animal study · PMID 37371128Review · PMID 41570941

How TB-500 is detected

The same 2012 study set out an analytical strategy for detecting Ac-LKKTETQ in plasma and urine using high-performance liquid chromatography with low-resolution triple-quadrupole mass spectrometry. Identification relied on the fragment's mass and chromatographic behaviour against the synthesized reference standard. In vitro · PMID 22962027

That work concerns the seven-residue fragment specifically. The mechanistic and animal studies in the sections above used full-length thymosin beta-4. The analytical paper reports how the fragment is identified; it does not test what the fragment does in cells or tissue. In vitro · PMID 22962027

What the TB-500 evidence does not settle

Most of the mechanistic work is decades old and used full-length thymosin beta-4, while the analytical literature describes TB-500 formulations built on the short fragment. Whether the fragment reproduces the actin-sequestering behaviour measured for the full peptide is not tested in the abstracts cited here.In vitro · PMID 1999398In vitro · PMID 22962027

The literature also points to more than one mechanism. The 2004 heart study described thymosin beta-4 as a G-actin-sequestering peptide, yet the effect it measured ran through a separate route: a functional complex with PINCH and integrin-linked kinase that activated Akt. Actin binding alone does not explain every result attributed to the peptide.Animal study · PMID 15565145

The 2026 kidney review lists the open problems for the full peptide as validating efficacy in more clinically relevant models, overcoming peptide instability and completing comprehensive safety assessments. It also proposes frameworks that allow for effects on fibrosis in both directions, because findings across injury models have not been consistent.Review · PMID 41570941

Frequently asked questions

Is TB-500 the same as thymosin beta-4?

Not necessarily. A 2012 analytical study identified the N-acetylated 17-23 fragment of thymosin beta-4, Ac-LKKTETQ, in a TB-500 formulation. The same name is also used for full-length thymosin beta-4, so the label alone does not establish which molecule a sample contains. In vitro · PMID 22962027Review · PMID 41570941

Is TB-500 the same as TB4?

TB4 usually refers to thymosin beta-4, the 5-kDa actin-sequestering peptide first isolated from calf thymus. TB-500 refers either to the full-length peptide or to its acetylated 17-23 fragment, depending on the source. In vitro · PMID 1999398In vitro · PMID 22962027

What does thymosin beta-4 do inside cells?

It binds actin monomers in a 1:1 complex and holds them out of filaments. In resting human neutrophils it is the major G-actin-sequestering protein, and after stimulation the bound fraction falls as actin polymerizes. In vitro · PMID 1999398In vitro · PMID 1447300

How is TB-500 identified analytically?

By liquid chromatography with mass spectrometry. A 2012 study identified Ac-LKKTETQ in a TB-500 formulation by high-resolution mass spectrometry, synthesized it as a reference, and proposed a triple-quadrupole method for plasma and urine. In vitro · PMID 22962027

Does TB-500 work in animal studies?

Animal studies report measurable effects, but in specific models. In a rat skin wound model, re-epithelialization was up to 61% greater at day 7. In a 2026 rat Achilles tendon study, load to failure was significantly higher. In mouse liver, however, endogenous thymosin beta-4 was linked to fibrosis rather than to repair.Animal study · PMID 10469335Animal study · PMID 42542926Animal study · PMID 37371128
